- 'Whether indended for (a) Private Use (b) Use for Trade Purpose (c) Use as a Public Conveyance';
- 'Date of Registration'.
Physical DescriptionVolume is in good condition, however pages 324-327 have been damaged and it looks like conservation has been undertaken. Even so, parts of the informational content are not legible.
