Description
1 letter of application for admission to the mental hospital as a voluntary patient and 1 letter from the Medical Superintendent confirming their admission to the hospital on 25 Feb 1926. This was Elizabeth Craine's 3rd admission to the hospital and included with her voluntary patient application are the following records:
- Patient reception order and medical certificate. This document records the admittance of the patient to the Mental Hospital, the reason for admittance and establishes the legal basis for their stay. Date of admission: 17 Jan 1917
- 1 letter of application for admission to the mental hospital as a voluntary patient and 1 letter from the Medical Superintendent confirming their admission to the hospital on 22 Dec 1924;
- 1 form titled 'Particulars Regarding Patient' which contains personal information and information about their condition. This form relates to their admission to the Mental Hospital on 22 Dec 1924.
